氧化石墨烯介导的光热免疫疗法治疗转移性小鼠乳腺肿瘤

摘要: 本研究分别从细胞层面和在体层面,研究纳米氧化石墨烯(nano graphene oxide,NGO)联合805 nm近红外激光对于肿瘤的杀伤和免疫刺激作用.细胞实验证实NGO具有良好的光热转换效应,联合激光治疗,能够有效杀伤肿瘤细胞.此外,NGO能够刺激巨噬细胞产生IL-6及TNFα,并增强激光杀伤的肿瘤细胞对巨噬细胞的免疫刺激效应.在体实验中,在小鼠背部不同位置使用同一瘤株种植两个瘤块,模拟局部原发肿瘤和转移瘤.实验结果表明,NGO联合激光治疗可以有效消融局部肿瘤,并且,未治疗的远隔部位肿瘤生长速度也显著降低.说明NGO联合激光治疗的方式在局部杀伤原位肿瘤的同时,可能诱发了机体的抗肿瘤免疫反应,从而达到了抑制远隔肿瘤生长的效果.

Photothermal-immunotherapy for Metastatic Breast Tumor in Mice Using Graphene Oxide

Abstract: The tumor killing and immune stimulation effects of photothermal therapy using nano graphene oxide (NGO)were studied in vitro and in vivo. For in vitro study, NGO showed a good photothermal conversion effect, increasing temperature under laser irradiation to destroy tumor cells. In addition, NGO could effectively stimulate the production of IL-6 and TNFα by macrophages and enhance the immune response of macrophages to laser-treated tumor cells. For in vivo study, two tumors were inoculated on both sides of the back of mice, serving as the local primary tumor and surrogate distal metastatic tumor. The results showed that the treatment of NGO combined with laser could effectively ablate the treated primary tumor, and inhibit the growth of untreated distal tu mor. It suggested that the treatment of NGO combined with laser therapy could stimulate the anti-tumor immune response of the body to attack distal metastases.

恶性肿瘤死亡率极高,且近年来发病率呈现出逐渐增高的总体趋势,已经成为现代社会人类健康的重大威胁[1].出现转移的恶性肿瘤患者失去外科手术机会,传统的放疗或化疗手段常不能达到有效的治疗目的,且伴随较为严重的副反应[2].因此,寻找和探索更加安全、有效且副作用低的治疗方案,具有十分重要的临床价值.

利用激光作为热源,对恶性肿瘤进行热杀伤治疗的方法,是一种新型的肿瘤治疗手段,受到国内外越来越多的关注.激光热疗常使用组织穿透性较好的近红外波段,但光热效果仍不理想.因此,有研究者针对激光热疗的这一局限性,提出联合使用光吸收材料,增强激光的光热转换效率,提高肿瘤局部热损伤,从而达到更好的肿瘤杀伤效果[3-4].

氧化石墨烯(graphene oxide,GO)具有特殊的理化及生物学特性,已在生物医学领域得到广泛应用[5].氧化石墨烯携带氧化功能基团,令其在水溶液中具有更好的分散性,此外,氧化石墨烯的细胞毒性极低,生产成本不高.以上这些特点,使得氧化石墨烯被认为是理想的高生物相容性材料,而应用于相关领域的研究中[6-7].研究发现,氧化石墨烯还具有较高的光热转换能力,特别是在近红外光波长区域,对光产生较强的吸收和热转换效应,产生的过高热能够直接杀伤肿瘤细胞[8].最新研究报道,氧化石墨烯能够有效刺激免疫细胞活化,诱发机体的免疫反应,并且聚乙烯醇化的氧化石墨烯能够显著增强这种免疫刺激效应[9].

恶性肿瘤的产生与发展过程,与机体免疫功能的变化有着密不可分的联系.而热治疗的效果不仅仅局限于高温对肿瘤组织的局部杀伤或与放化疗等传统治疗的协同效果[10-12],热治疗本身还可能刺激机体免疫系统,联合使用免疫佐剂,还可以诱发特异性的抗肿瘤免疫反应,有效抑制远端转移瘤[13-15].

本研究使用氧化石墨烯联合805 nm近红外激光治疗,观察其在肿瘤治疗中的作用.

1 材料与方法 1.1 氧化石墨烯的制备

1 g石墨烯(XFNANO,China)加入23 ml 98%的H2SO4混合8 h.保持温度<20℃,缓慢加入KMnO4(3 g).混合后在35℃~40℃下搅拌30 min,在65℃~80℃下搅拌45 min.然后加入46 ml水,100℃加热混合物30 min.加入140 ml蒸馏水和10 ml 30% H2O2溶液中止反应.依次使用5%的HCl水溶液和蒸馏水反复地离心和过滤清洗混合物.

1.2 纳米氧化石墨烯(nano graphene oxide, NGO)的制备和表征检测

10 ml(1 g/L)的氧化石墨烯溶液中加入1.2 g NaOH.溶解3 h后,加入盐酸,直到pH值下降到1.用水冲洗3次,浓度控制为1 g/L.溶液中加入氨基化六支链乙二醇(6PEG-NH2),浓度为2 g/L,超声粉碎5 min.加入1-(3-二甲氨基丙基)-3-乙基碳二亚胺盐酸盐(EDC,Sigma-Aldrich,USA) 1 mmol,超声粉碎1 h.之后使用2倍的磷酸盐缓冲液,以12 000 r/ min的速度将氧化石墨烯溶液离心分离,去除聚合体和多层的石墨烯.离心分离和清洗8次后,在100 kda MWCO离心过滤器(微孔)中过滤,获得本研究所使用的经氨基化六支链乙二醇处理的NGO.使用紫外可见近红外分光光度计检测NGO在600~1 100 nm的吸收光谱,测量峰宽为5 nm,测量速度为200 nm/min.使用原子力显微镜及动态光散射仪检测纳米石墨烯的形态和尺寸特征.

1.3 NGO溶液的光热性能检测

在300 μl的EP管中分别加入100 μl浓度为40 mg/L的NGO溶液,使用805 nm激光器,在0.5~2.5 W/cm2不同功率下照射5 min,使用红外热成像仪记录温度(间隔30 s).在300 μl的EP管中分别加入100 μl不同浓度NGO溶液(0~80 mg/L), 在805 nm激光器功率为2 W/cm2条件下进行照射5 min,并使用红外热成像仪记录温度(间隔30 s).

1.4 细胞培养

小鼠乳腺癌细胞4T1(ATCC,USA)以含10%胎牛血清(ATCC,USA)的RPMI1640培养基(GIBCO,USA),在37℃含5% CO2的培养箱内培养.小鼠巨噬细胞RAW264.7(ATCC,USA)以含10%胎牛血清(ATCC,USA)的DMEM培养基(GIBCO,USA),在37℃含5% CO2的培养箱内培养.

1.5 细胞杀伤实验

在4T1细胞中分别加入不同浓度的NGO溶液,在不同剂量近红外激光下照射5 min,24 h后检测肿瘤细胞的存活率.细胞活性检测:每孔加入10 μl的MTS(Promega,USA)继续培养2 h后,用酶标仪检测490 nm处各孔的吸光度(A)值.以实验平行的单纯培养液的空白孔调零,记录A值,每个实验重复3次,取其平均值,细胞的存活率可以表示为:

细胞死亡检测:处理后的细胞进行FITCAnnexinV/7AAD(Millipore,Germany)双染,之后通过流式细胞技术,分析肿瘤细胞的杀伤效果.

1.6 酶联免疫吸附检测(ELISA)分析

在巨噬细胞RAW264.7(2×105)中分别加入不同浓度的NGO,24 h后使用ELISA法检测细胞上清液中IL-6及TNFα(R&D,USA)的浓度.4T1(2×105)细胞分为三组进行不同处理:空白对照组、805 nm激光(2 W/cm2,5 min)组、805 nm激光(2 W/cm2,5 min)联合40 mg/L NGO组、将处理过的肿瘤细胞分别与巨噬细胞(2×105)共培养,24 h后用ELISA法检测细胞上清液中IL-6及TNFα的浓度.

1.7 肿瘤治疗

使用雌性Balb/C小鼠,清洁级,鼠龄6~8周.于小鼠背部两侧同时种植小鼠4T1乳腺癌细胞,肿瘤体积长至约100 mm3时治疗左侧肿瘤.激光治疗组使用805 nm近红外激光(2 W/cm2, 5 min),激光+NGO组局部注射NGO(1 mg/kg),6 h后照射805 nm激光(2 W/cm2,5 min),利用红外热成像仪记录治疗过程中肿瘤表面温度.治疗后观察左右两侧肿瘤体积变化.测量方法为用游标卡尺按直角测量肿瘤的长轴和短轴,最后计算肿瘤体积V=ab2/2,a为长轴,b为短轴.

2 结果 2.1 NGO物理学特性分析

石墨烯是以sp2混成轨域呈蜂巢晶格(honeycomb crystal lattice)排列构成的单原子层的二维晶体纳米材料.分光光度计检测结果表明,NGO在600~1 100 nm之间形成了吸收曲线,证明在这个波长区间内NGO存在着光吸收能力(图 1).

为了进一步了解NGO的形态特征及尺寸,我们使用原子力显微镜及动态光散射仪对NGO进行检测.原子力显微镜结果显示,NGO呈现片状,分散结构,如图 2a.动态光散射结果显示NGO的直径范围在40~60 nm出现最大峰值,并且在200~400 nm出现另一个较低的峰值,如图 2b.

2.2 NGO光热转换效应分析

为了进一步证实NGO的光热转换性能,我们检测了NGO溶液在激光照射下的温度变化.如图 3a所示,相对于水溶液,NGO溶液的温度在同样光剂量照射下显著升高,说明NGO具有较好的光热转换能力.进一步分析NGO溶液的光热转换效果,发现同样浓度NGO在不同光剂量照射下温度增长程度不同(图 3b),且同样光剂量照射下,不同NGO浓度也引起不同的温度增长(图 3c).说明NGO的光热转换效果依赖于NGO浓度和光剂量.

2.3 光热杀伤效应的细胞学实验

使用不同功率的激光联合NGO(40 mg/L)处理4T1细胞.结果显示,伴随着激光功率的升高,激光联合NGO治疗对肿瘤细胞的杀伤效果逐渐增强,在2 W/cm2的功率下,达到较大的杀伤效果.而单纯使用激光治疗,对肿瘤细胞的杀伤效果均不明显(图 4).说明单纯的805 nm激光照射,对于肿瘤细胞活性的影响十分有限,联合NGO,能够有效地增强光热转换,达到细胞杀伤的效果.

在以上实验结果的基础上,对4T1细胞使用功率2 W/cm2的805 nm激光照射联合20 mg/L及40 mg/L两种浓度的NGO治疗,通过流式细胞技术,分析肿瘤细胞的杀伤效果.结果显示,相对于对照组及单纯激光治疗组,激光联合NGO的治疗方式,能够有效杀伤细胞.相对于20 mg/L浓度的NGO,40 mg/L浓度的NGO能够获得更好的肿瘤细胞杀伤效果(图 5).

2.4 NGO及联合治疗的免疫刺激效应

细胞实验结果显示,在巨噬细胞溶液中加入不同浓度的NGO后,伴随NGO浓度的增高,免疫相关细胞因子肿瘤坏死因子α(TNFα)和白介素6 (IL-6)的浓度逐渐升高(图 6a).结果说明NGO具有浓度依赖性的免疫刺激作用.

当我们将不同治疗方式处理过的肿瘤细胞分别与巨噬细胞共培养,激光联合NGO组细胞因子释放的水平显著高于空白对照组及单纯激光治疗组(激光功率2 W/cm2,NGO浓度40 mg/L).结果说明,在细胞水平,激光联合NGO的治疗方式,可以有效地刺激巨噬细胞活化(图 6b).

2.5 原位及远隔部位肿瘤杀伤的在体实验

在肿瘤在体实验中,与单纯激光治疗组相比,激光联合NGO治疗能够显著增加肿瘤部位温度(图 7).

图 8a所示,在小鼠背部两侧同时种植肿瘤,仅治疗左侧肿瘤,之后观测治疗部位肿瘤生长及右侧未治疗部位肿瘤生长情况.参考既往相关研究结果,动物实验治疗使用NGO的剂量为1 mg/kg[16-17].背部左侧肿瘤生长曲线显示,与空白对照组比较,激光治疗组轻度抑制了肿瘤的生长,然而激光联合NGO治疗组显著抑制了肿瘤的生长.从肿瘤生长曲线分析,3 d后,激光联合NGO治疗组肿瘤开始缩小,而对照组及激光治疗组肿瘤体积继续增长.15 d时,激光联合NGO治疗组肿瘤基本消除,而对照组及激光治疗组肿瘤体积达600 mm3以上(图 8b).结果表明,激光联合NGO能够产生显著的光热效果,有效杀伤肿瘤.

背部右侧肿瘤生长曲线显示,与空白对照组及单激光治疗组比较,激光联合NGO治疗组显著地抑制了肿瘤的生长.从肿瘤生长曲线分析,15 d时,激光联合NGO治疗组肿瘤体积维持于300 mm3左右,而对照组及激光治疗组肿瘤体积达500 mm3左右(图 8c).激光联合NGO的治疗并未直接作用于小鼠背部右侧肿瘤,然而背部右侧肿瘤的生长受到明显抑制.结果表明,激光联合NGO的光热治疗,可能激发机体的免疫反应,从而使远隔部位的同种肿瘤生长受到抑制.

3 讨论

转移性恶性肿瘤在临床上具有难治性高和致死率高的特点[18-19].手术治疗方式常常不适用于伴有远隔部位转移的晚期肿瘤患者.放疗和化疗等传统治疗方式,又由于副反应高,患者不耐受等原因,在临床使用中受到很多限制[20-21].因此,寻找新的、低毒、高效、并能对转移性肿瘤同时产生治疗效果的新治疗策略,对于晚期肿瘤的治疗具有重要的意义.

NGO具有良好的光热转换效应,联合激光治疗,能够有效产生过热效果,从而达到肿瘤杀伤的目的[22].光热杀伤细胞实验结果表明,在NGO联合激光治疗策略中,对于激光功率和NGO浓度都存在着剂量依赖效应:伴随着激光功率和NGO浓度的增高,肿瘤细胞的杀伤效应也得到显著提高.

并且我们在细胞实验中观察到,NGO溶液可以有效地刺激巨噬细胞产生IL-6及TNFα.巨噬细胞参与先天性和适应性免疫反应,如作为吞噬细胞,抗原呈递细胞和延迟型超敏反应的效应细胞[23].本研究所使用的RAW264.7是一种永生化巨噬细胞,由于表征稳定,被广泛应用于体外的巨噬细胞研究[24-25].IL-6及TNFα是免疫细胞控制和影响癌细胞增殖能力的重要细胞因子,因此被作为研究抗肿瘤免疫反应的常用指标[26-27].相比于对照组及单纯使用激光组,NGO联合激光治疗方法可以有效地促进巨噬细胞产生IL-6及TNFα.这些细胞因子是巨噬细胞抗肿瘤型分化的特征性因子,因此提示NGO联合激光治疗方法可以有效地刺激抗肿瘤免疫反应的产生[28-30].

在体实验表明,相对于单纯使用激光治疗,NGO联合激光的治疗方式能够形成更理想的光热反应,产生更高的热效应,有效地损伤肿瘤组织.另外,在NGO联合激光治疗组中,虽然远隔部位的肿瘤从未接受过任何局部治疗,但其生长速度却显著低于对照组及单纯激光治疗组.结合细胞学实验结果,我们可以推论:NGO联合激光治疗的方式在局部杀伤原位肿瘤的同时,可能诱发了机体的抗肿瘤免疫反应,从而达到了抑制远隔部位肿瘤生长速度的效果.但这一过程是否真正合并了具有治疗意义的抗肿瘤免疫反应,以及产生了哪一种类别的免疫反应,还需要进一步的研究加以证实.
