深部脑刺激作用机制的研究进展

摘要: 深部脑刺激(deep brain stimulation,DBS)已在临床上广泛用于治疗帕金森病等疾病引起的运动障碍,它在难治性癫痫、顽固性强迫症等其他脑中枢神经系统疾病的治疗上也展现出良好的应用前景.经过30多年的临床应用、动物实验和计算模型仿真等多方面的研究,DBS的机制也逐渐明朗.虽然尚无定论,但已取得许多重要进展.本文从电生理角度分析和总结了有关DBS机制的发展历程.从早期的抑制论和兴奋论到目前主导的调控论;从关注刺激位点的神经元活动,到发现神经元胞体与轴突活动的去耦合,再到高频刺激诱导的间歇性轴突阻滞,以及由此轴突活动可能导致的投射区神经元群体的去同步活动.这一系列研究进展表明DBS具有复杂的神经网络调控机制.了解DBS的作用机制对于提高其疗效、开发新刺激模式以及扩大临床应用的范围都具有重要意义.

Advances in Understanding The Mechanisms of Deep Brain Stimulation

Abstract: Deep brain stimulation (DBS) has been applied widely in clinic to treat movement disorders such as Parkinson's disease. It also shows great prospects in the treatments of intractable epilepsy, refractory obsessive-compulsive disorder, and other diseases of the central nervous system in brain. Over the past three decades, the mechanisms of DBS have emerged gradually based on multiple lines of evidence in clinical applications, in animal experiments, and in simulations of computational models. Important advances in DBS mechanisms have been achieved although final conclusions are still under debate. This review analyzes and summarizes the development of DBS theories from electrophysiological perspectives: from the original theory of inhibitions or excitations to the recent prevalent theory of modulations; from focusing on neuronal activity locally at stimulation sites to discovering the decoupling of somatic and axonal responses, and further to discovering the intermittent depolarization block of axons induced by high frequency stimulations, together with de-synchronous activity presumably caused by axonal activity in population neurons in the projection brain regions. The series of advances indicate that DBS has complex mechanisms in modulating the neuronal networks. Understanding the mechanisms of DBS has significance for improving DBS therapies, for developing new stimulation modes, and for extending its clinical applications.

深部脑刺激(deep brain stimulation,DBS)的临床应用始于1980年代,通常认为法国格勒诺布尔大学(University of Grenoble)的神经外科学家Benabid等人的工作是里程碑.在利用DBS控制帕金森病患者的症状时,研究者们发现电脉冲高频刺激在治疗震颤、肌强直、运动迟缓等运动障碍时可以产生类似于损毁术的治疗效果[1-2].损毁术永久性破坏病灶脑区的神经组织,可能造成后遗症;而DBS仅在施加电刺激时对神经系统产生作用,具有副作用小、电极植入手术微创、治疗作用可控可逆、并发症少等特点.因此,目前在帕金森病等运动障碍疾病的治疗中DBS已成功替代了大多数损毁手术.此外,DBS在治疗其他脑神经系统疾病,如难治性癫痫、毒品成瘾、顽固性强迫症等神经与精神疾病方面,也展现出良好的应用前景[3-4].甚至在治疗智力障碍、改善脑的记忆功能、唤醒脑的植物状态等方面也获得了令人瞩目的研究成果[5-6].

DBS在国际上的临床应用已有30多年历史,国内引进这项技术也有20余年,目前全国数10所医院可以开展该项治疗[7-9].在DBS技术的发展历史中,临床经验起着重要作用.具有临床应用在先,机制研究在后的特点[10].有关DBS的作用机制至今尚无定论.不过,多年来,大量临床试验、动物实验以及计算模型的仿真研究等获得的结果正在不断推进对于DBS机制的认识.

关于DBS机制的假说大致可以分为抑制、兴奋和调控三大论点.a.抑制论:作为神经外科损毁术的替代方法,早年DBS的作用机制被推测为抑制神经元的活动.b.兴奋论:动物实验和数学模型仿真等许多研究结果表明DBS具有兴奋性,可以提高某些神经元的动作电位发放率.c.调控论:DBS可以调节神经元动作电位的发放模式,改变发放节律等,进而产生复杂的调控作用.本文下面介绍这几种假说的发展历程.

1 刺激部位的神经元活动抑制论

目前DBS较成功的临床应用是治疗帕金森病.早在1990年代,DBS技术发展的初期,大脑皮层活动的过度抑制曾经被认为是帕金森病的重要病因之一.也就是,苍白球内侧部(internal globus pallidus,GPi)和/或黑质网状部(substantia nigra pars recitulata,SNr)的抑制性神经元的发放增加,抑制了其投射区丘脑的神经元活动,减弱了丘脑对于大脑皮层运动区的兴奋性激励[11].由于丘脑底核(subthalamic nucleus,STN)对于GPi和SNr具有兴奋驱动作用;因此,损毁STN或者损毁GPi都可以缓减大脑皮层的过度抑制,进而缓减帕金森病的症状.

DBS治疗帕金森病的常用刺激部位就是STN和GPi[12],而且其治疗效果与损毁术相似.因此,研究者自然就推测DBS的可能机制是减弱了这些刺激部位的神经元活动.猴和大鼠等动物的电生理实验结果也表明,STN和GPi核团的电刺激确实可以减少刺激电极周围神经元的动作电位发放[13-14].于是这种推测似乎得到了证实,这就是早期有关DBS机制的抑制论.那么,电刺激的抑制作用是如何产生的呢?

常规DBS采用的是脉宽为0.1 ms左右的窄脉冲序列的高频刺激(high frequency stimulation,HFS),临床治疗帕金森病震颤的有效刺激频率范围为90~185 Hz[15-16].如果施加独立的单个电脉冲,那么刺激对于细胞膜具有兴奋作用,可以诱发神经元产生动作电位.然而,DBS的HFS脉冲序列却表现出与之相反的抑制作用.可见,其中可能包含复杂的机制.起初,研究者们主要根据刺激电极周围的局部区域中神经元细胞膜发生的变化来推测其中的机制,并指出:a.HFS密集的兴奋性输入可能导致细胞膜产生持续的去极化,致使膜上的电压门控钠离子通道失活,产生所谓的去极化阻滞(depolarization block),使得神经元无法继续产生动作电位[17-18].b.刺激可能激活了胞体周围的抑制性突触的突触前轴突(或轴突终末),从而增加了抑制性神经递质的释放(如GABA等),使得突触后的神经元受到抑制[14, 19].

这两种机制似乎可以解释刺激部位神经元动作电位发放的减弱.但是,仍然存在一些疑问. a.这两种机制是相互对立的,一种使得细胞膜去极化,而另一种则使细胞膜超极化.如果同时发生于同一个神经元,那么两者的作用可能会相互抵消.b.刺激可能会同时激活连接抑制性突触和兴奋性突触的轴突,因此,神经元究竟是被抑制还是被兴奋,应该取决于两种作用平衡的结果,不一定是被抑制.有些临床实验记录也确实发现神经元的发放并没有被刺激减弱[20].

而且,许多现象无法用抑制论来解释.例如,猴类帕金森病模型的实验研究显示,STN刺激期间,虽然刺激部位STN的神经元发放减少,但是其投射区GPi的神经元发放不是减少,而是增加.与此同时刺激的作用仍然是有效的,可以缓减运动障碍的症状[21-23].此外,临床DBS的治疗结果表明:电刺激不仅对于大脑皮层活动过度抑制导致的运动减退有效,而且对于肌张力障碍等的运动亢进也有效[4, 22].可见,这些疑问动摇了单纯的DBS抑制论[24].

更值得注意的一个问题是,早期的有些研究是根据刺激部位局部区域的神经元活动(即动作电位发放的增减)来推测它们对于下游投射区域的作用.如果这样,首先需要具备一个前提条件,那就是,神经元胞体产生的动作电位与沿其轴突外传的动作电位是一致的.这对于正常生理状态的神经元是不言而喻的.但是,DBS时的情况并非如此,这个前提条件可能不成立.DBS所使用的高频刺激(HFS)会使神经元的胞体与其自身轴突的活动相互脱节,使两者产生不一致的活动.即便胞体被抑制,不能产生动作电位,这种刺激仍然可以使其轴突产生动作电位并传导出去.这就是所谓的“当地抑制,远处兴奋”的现象[25-26].

2 刺激位点神经元胞体活动与其轴突活动之间的去耦合

在DBS这样的细胞外电脉冲刺激作用下,由于轴突的兴奋阈值比较低[27],因此,它比胞体和树突等神经元的其他结构更易于被激活.无论胞体的状态如何(正常、过度兴奋或者过度抑制),刺激都可能在轴突上独立地诱导出动作电位,并向两侧传导.逆向(antidromically)传向胞体的动作电位会与胞体的活动相互作用,干扰胞体的原有活动,并阻止原有活动外传.而顺向(orthodromically)传向轴突终末的动作电位则会影响轴突和终末突触的状态,这些部位的状态可以与胞体状态无关.这是神经元胞体活动与轴突活动之间去耦合的理论基础.

动物实验研究确实发现,高频刺激可以在抑制胞体的同时兴奋其轴突,使轴突产生动作电位[28].计算模型的仿真研究结果也表明,在胞外HFS期间,神经元胞体的活动并不一定与其轴突上传出的信息一致[29].胞体受到抑制性突触的作用而被抑制时,轴突却可以跟随刺激脉冲产生动作电位并传导出去,从而导致胞体与轴突活动的“去耦合”.这种去耦合使得轴突上高频刺激诱导的活动替代了原本来自胞体的兴奋性活动.

轴突上刺激诱导的活动可以通过单突触和多突触的传递,投射至多个脑区.可见,相比于刺激部位局部区域神经元胞体的活动,高频刺激在传出轴突纤维上诱导的活动对于DBS的疗效更为重要.许多实验研究表明这些输出可以兴奋下游投射脑区的神经元[30-32],这支持DBS的兴奋论.但是,刺激位点的兴奋性输出似乎有悖于DBS类似损毁术的疗效.近年来有关轴突高频刺激的研究结果为解决此矛盾提供了新线索.

3 高频刺激可以诱导轴突阻滞

脑内轴突纤维无处不在,其所占据的空间远大于胞体、树突等神经元其他成分的总和[33].在刺激电极附近直接受到刺激作用的局部区域内,既存在来自上游神经元的传入轴突,也存在本地神经元的传出轴突,还存在“路过”的轴突纤维.这些轴突都很容易被刺激激活.因此,近年来轴突在DBS中的作用越来越受到重视[15, 34-35].特别是高频刺激(HFS)诱导轴突阻滞现象的发现为揭示DBS机制展开了新篇章.

离体脑片上丘脑和基底神经节区域的高频刺激实验表明,轴突无法跟随50 Hz以上的每个刺激脉冲产生动作电位,会出现轴突传导的阻滞现象[36-38].利用海马脑区清晰的层状结构和轴突层与胞体层之间的明确分界,在体动物实验也证实高频刺激可以诱导轴突阻滞[39-40].此外,大脑皮层神经元的轴突纤维上也存在HFS诱导的轴突阻滞[35].可见,HFS诱导的轴突效应广泛存在于不同脑区.当然,不同脑区神经元轴突结构的差别(如轴突的粗细、有无髓鞘等),可能会影响刺激诱导轴突阻滞的程度及其发生和发展的动力学过程[35],这些问题还有待于深入研究.

高频刺激HFS诱导轴突细胞膜产生阻滞的机制可能是胞外钾离子浓度([K+]o)的升高[26].HFS起始时诱导的高频动作电位发放可以迅速升高[K+]o,致使细胞膜产生动作电位后延续去极化状态,不能及时复极化,无法连续响应高频的脉冲刺激[41-42].大鼠海马脑区实验研究表明,人为提高[K+]o,也可以导致轴突传导失败,产生这种去极化阻滞[43].而且,轴突纤维束内的胞外空间狭小,细胞膜去极化时外流的钾离子容易积累,不易扩散,易于导致去极化阻滞[44].还有,轴突分叉、HFS引起的轴突形态结构和生物物理特性的改变等因素也可能导致轴突传导的阻滞[35, 45].有关HFS诱导轴突阻滞的机制尚待深入研究,才能获得比较明确的定论.

此外,HFS诱导的轴突活动传向轴突终末时,也会影响终末的状态,特别是可能影响终末突触前的神经递质的产生和释放等过程.例如,HFS初期诱导的强烈的轴突兴奋活动[32, 40],传导至终末的突触时可能诱发神经递质的过度释放,引起神经递质的耗竭,进而导致突触传递失效,限制并减弱HFS诱导的兴奋作用的传播[13, 46].不过,随着HFS的持续,等到轴突上发生阻滞之后,刺激作用受限制的部位可能就前移至终末之前的轴突膜上,终末的突触传递就可能恢复.鉴于目前临床治疗通常使用长时间持续的高频刺激,刺激的兴奋作用受限于轴突阻滞的可能性要高于突触传递失效的可能性.但是,不能完全排除突触传递失效(如神经递质耗竭等)的存在,也可能轴突阻滞和突触失效两者并存.或者在高频刺激的不同时期,两者的主导地位会发生更替.因此,本文下面所述的“轴突阻滞”不排除轴突终末突触失效的作用.值得注意的是,这种多因素的共同作用更有利于开发多样化的DBS调控作用,以满足不同脑疾病治疗的需求.

脊髓和外周神经上利用高频电刺激来阻断轴突的神经信息传导,可以获得镇痛等治疗效果[47].借鉴此类现象,脑内HFS诱导的轴突阻滞的主要作用也曾经被认为是阻断轴突两端之间的兴奋传导[38, 40].而且,实验研究也证明这种轴突阻滞可以阻断传导:在海马脑区的轴突纤维上分离安置两个刺激电极,当其中一个电极上施加HFS诱导轴突阻滞时,另一个测试电极上单脉冲诱发的兴奋就不能穿过HFS作用的部位,形成了功能性的“断开”效应[39].

这种HFS诱导的轴突传导阻滞可能是DBS的重要机制之一[26, 37].而且,它的“断开”效应又一次使DBS的作用与损毁术接近了.但是,除了“断开”刺激部位上游与下游之间的神经信息传导之外,这种轴突阻滞在DBS中可能还发挥着其他重要作用.

4 轴突活动对于神经元的调控作用可能是DBS的一个重要机制

高频刺激可以诱导轴突阻滞,其导致的效应并不仅限于阻断神经信息的传导,它可能具有如下多重作用:

a.轴突阻滞并不意味着受控制的下游突触后神经元就失去了来自这些轴突的激励.数千赫兹的更高频率的刺激才能使得轴突纤维持续保持去极化阻滞,即完全阻滞[48];而DBS采用的低于200 Hz的刺激频率只能产生“部分性”的阻滞[39, 46, 49],或者称为“间歇性”的阻滞.也就是,HFS仍然可以在轴突上以低于刺激脉冲的频率诱导动作电位,并将兴奋传递至轴突终末,经过突触传递,再作用于突触后神经元[31-32, 50].

b.通过间歇性轴突阻滞,高频刺激所产生的激励可以在轴突的投射区建立新的神经活动,以取代原有的活动.并且这种新活动具有去同步性[20, 32], 可以消除神经元原有的爆发式发放[31, 50],或者抑制神经元及其网络的节律性活动等[51-53].

c.发生轴突阻滞时,刺激本身在轴突上的作用是被部分阻滞,但是,来自上游神经元的信息却可能被完全阻断.这是因为,实验研究显示:HFS期间下游神经元的动作电位发放率增加,而刚撤除HFS时,这些神经元却会立刻停止发放,静息数十秒之后才逐渐恢复发放[32].此现象说明,HFS期间下游投射区神经元的活动完全由刺激驱动,不包含上游神经元传下来的驱动.否则,刺激撤除后,上游神经元的驱动作用会维持下游神经元的部分发放.上游神经元无驱动下传的可能机制是:HFS产生的间歇性驱动在轴突上可以双向传导,逆向传向连接在轴突上的胞体时,也会调控胞体,使得它们按照刺激的驱动产生活动[40, 49],压制它们原有的活动.一旦失去HFS的控制,需要经过一段时间之后,上游胞体才能恢复原有活动,对下游产生驱动.这段恢复时间就导致了刺激刚撤除时下游神经元的“静息期”.

由此可见,轴突HFS产生的去极化阻滞可以切断上游与下游之间的神经信息传导,从而中断病理性的异常神经信号的传播[54],与此同时,刺激脉冲又可以调控上游和下游神经元的活动.HFS期间刺激脉冲诱导的驱动对于下游神经元及神经网络的作用可能是DBS发挥作用的关键,而间歇性的轴突阻滞和恢复使得这种刺激的驱动作用产生了随机性,减弱了其诱发的动作电位与刺激脉冲之间的锁相性,也就是减弱了各轴突之间兴奋的同步性,因而导致下游脑区神经元群体的去同步活动[26, 32, 37].这种HFS诱导的新活动可以替代并消除原有的病理性同步活动,例如:帕金森病的β节律(目前认为这种节律活动才是其重要病因)[3, 31],或者抑制癫痫等疾病引起的过度同步活动[55-56],从而产生DBS的疗效.这就是目前占主导地位的DBS调控论.

5 结语

多年来的研究成果使得有关DBS机制的假说发生了重要变化,从抑制为主的单纯作用变成了调控为主的复杂作用[25-26],相关研究的关注点也从神经元动作电位发放的增减,扩展到动作电位发放的模式、同步性和节律性等更多的考察指标[23, 30, 57].不过,从电生理学的角度来看,DBS产生调控作用的基本机制离不开高频刺激(HFS)的脉冲对于细胞膜的兴奋作用.这种兴奋可能诱导胞体和轴突细胞膜的间歇性去极化阻滞,进而以去同步的方式驱动下游脑区的神经元活动,改变病理性神经活动.DBS的这种调控方式看似与“损毁术”的作用不同,但两者也可能存在共性.如果从信息传输的角度来看,刺激期间神经元的活动受控于刺激,无法传导生理性的自发活动,原本的神经信息传导被阻断.这种“信息破坏”也是一种“损毁”[58-59].

虽然有关DBS机制的研究已经取得重要进展,但是许多机制仍然有待于深入研究.例如,“调控论”表明DBS引起的神经元及其网络的改变不是单纯的兴奋性或者抑制性,会随着不同疾病所导致的不同脑神经状态而变化,对于这些变化的认识还亟待深入.而且,DBS还会影响胶质细胞的活动,如引起胶质细胞释放谷氨酸和腺苷,进而间接改变受刺激核团神经元的兴奋性[60-61].此外,DBS还可能促进某些神经递质(如多巴胺等)的释放,也可能通过突触的可塑性变化产生神经网络的重组,或者导致神经再生,产生轴突等结构的形态变化等,从而产生更复杂的作用[25, 45, 62].有关这些机制的深入研究将有利于推进新型DBS治疗模式的开发,并不断扩大其临床应用的范围.
